May I suggest another factor, a cumulative multiplier called Reappointments Required (RR). Usually accompanied by "OK, lets do a few tests and then see what we can do for you...." A treat for predicting the income of the practise and analysing the investment opportunity.... in the stock, not your personal investment.
So in your example above case 2. Adult onset diabetes, good for at least 5-10 visits, so visit one may be DOWI =0.025, visit two 0.025 etc In two visits CDOWI would be 0.000625 or in 5 visits 9.76 x 10-9 (10 to the minus 9, sorry no superscript). An index cound be derived on the cumulative power of ten and onsold to the market. Derivatives in medical practises would then be devised on the rise or fall of the index for individual practises and long or short selling encouraged by the owners of the superannuation (401) funds.

I must say I like it, but feel that it should be a separate index of its own, given the inability of most folks to deal with very small numbers. Perhaps you could make it log scale like the Richter but use your Reappointments Required or something a little more catchy...
